New Patients Start Here
Below is a Walk through of what it's Like when you call our office and become a patient at our Office.
Dr Sam and the entire team are excited to meet you and help you get better.
How to Get Started at our Office:
  • First: Call our office at (555) 555-5555 to schedule an exam with the Doctor.
  •  Second: Print and fill out our intake forms and bring them to your first visit.
  •  Finally: Show up 10 minutes prior to your appointment time and bring your Identification Card and Proof of Insurance (if you aren't paying by cash). We can verify your insurance cover when you initially schedule your appointment.
Some Questions We Get Asked Often:
Our priority is to make sure that we help you address your condition as safely as possible. At your first visit, here's what you can expect:
What Can You Expect When You Walk into the Office for the First Time?
Our front desk staff will great you and take your paperwork. They'll also ask you for your Identification card and your insurance card so that they can make copies for the office's records.
How Long does it Take To See the Doctor the first time?
We do our very best to schedule your first appointment during a time when our normal patient load is light so that the Doctor can spend more time with you and learn about you and your condition (This will also depend on your schedule). Once we have all of your initial paperwork done, our staff will take you back to a private room where you'll wait for the Doctor (standard wait times in our office are 5-10 minutes).
Does the Doctor Do an Exam?
Yes. An exam is essential to learning about you, your condition, and being able to treat your safely and efficiently. During your first visit, the Doctor will ask you questions about your medical history and the condition that you are coming in for. These questions will be followed by a focused exam that will help us determine a diagnosis and what the cause of condition is.
Do you have to get X-Rays?
We don't perform X-Rays on every patient that enters our office. The decision to order X-Rays (or other advanced imaging) is based on the results of the initial exam. We only perform X-rays when they are needed to form a Diagnosis or to rule out anything that might be dangerous to you.
What kind of treatment will I be receiving at my first visit?
Treatment is different for every patient.

Once the Doctor is able to diagnose the condition from the exam findings, you will be give a report of findings. During this report your condition, the Doctor's Recommended treatment plan, and your treatment alternatives will be presented. No Treatment will be Performed until your understand your condition, options, and have given your permission.
Is it True that once you see a Chiropractor, you have to keep coming to see them forever?
Absolutely not.

You will be given a specific treatment plan based on your exam findings. After your exam and report of findings, you will know exactly how long it will take to treat your condition and will be given the choice to continue before we start on that plan. Our goal is to keep you out of the Doctor's office by getting you well and by helping you to avoid the things that caused the condition in the first place.
Do I have to get my Neck Adjusted?
Nope.

We don't treat any areas that don't call for treatment. In addition to that, you are in charge. The Doctor will present what he thinks needs to be done, but you are always in charge of your body and care.
